In order to establish an entity account with CM Capital Services the following is required:

• Proof of valid established entity
• Copy of Operating Agreement or other such document providing for ability to establish and direct account
• Verification of signature authority for the entity account
• Completion of standard CM Capital Services investor documentation to create an account
